To Nexus Emergency Vet*, please complete the referral form below. This helps us ensure we receive all relevant clinical information, allowing for a smooth transition of care and the best possible outcome for your patient.

Once we receive your referral, we will send an SMS to the client with a check-in form to make the transition smooth and call you if further information or a direct vet-to-vet handover is required.
